Health · cover

Different is not automatically better.

Statutory and private health insurance follow different rules. The question is not which system sounds more premium, but which structure fits work, family, health priorities and long-term affordability. Before comparing tariffs, the questions that matter in a claim come first.

How it works → — Understand first. Then decide.

Two parallel paths: statutory and private health insurance as a diagram.

The real question

What matters when medical care is needed?

A tariff name does not answer that. Choose a question — the note stays with what would be needed in a claim, not with a product.

What can matter when care is needed

Choose a question. Nothing here becomes a recommendation or an application.

Free choice, a GP pathway, or a mix. The difference shows with a specialist, not when a brochure prints 100 percent.

Shared, double or single room, ward or private physician — layers. Wanting one does not mean the whole contract fits.

Treatment, prophylaxis, tooth replacement, implants and orthodontics are different questions. Early-year scales can make bills smaller than the percentage sounds.

Sessions, prior approval, sometimes a GP pathway. A percentage without conditions is not a description of cover.

In private cover an invoice is often submitted, with upfront payment, receipts and a deductible.

Partner, children, a child still to come. Statutory cover often includes family insurance; private cover is usually one contract per person.

The picture

Health insurance is a puzzle, not a single promise.

Outpatient care, hospital, dental treatment and contract rules belong together. Building blocks by area — in text, without choosing a tariff.

Outpatient
Doctor
Who provides treatment and whether a GP opens the way to a specialist.
Remedies
Physiotherapy and similar prescriptions — often up to a tariff list.
Fee schedule
Invoices follow fee schedules; going beyond maximum rates is tariff-specific.
Psychotherapy
Session tiers and prior approval — without conditions the percentage is incomplete.
Prevention
Check-ups and vaccinations; often outside the deductible and treated differently for a premium refund.
Hospital
Hospital physician
Ward, affiliated or private physician — “hospital” without that layer says little.
Room
Shared, double or single; a single room is often its own layer.
Hospital choice
Free choice in some tariffs; prior approval can still be required.
Dental
Dental scale
Early years limit tooth replacement and implants; orthodontics is separate in the tariff.
Dental treatment
Fillings and check-ups — a different line from a crown or implant.
Tooth replacement
Crowns, bridges, dentures; materials and lab often follow a price schedule.
Rules of the contract
Deductible
The yearly share to carry — a lower premium does not erase that gap.
Treatment abroad
Europe and the rest of the world are different sentences; residence and duration matter.
Premium refund
The board and trustee set form and amount each year — not a promise on a future premium.

Two systems

Statutory and private insurance are different orders.

Statutory health insurance is a solidarity system with its own strengths. Private health insurance is a contract with its own duties. Neither is the morally better choice.

Not “Is private better?” but which structure fits — today and over the long term.

Logic

Statutory health insurance

Solidarity. Contributions follow income. Benefits follow statute and shared rules.

Private health insurance

A contract. Premium and benefits follow tariff, age at entry, health at application and what is agreed.

Family

Statutory health insurance

A spouse and children can be covered through family insurance when statutory conditions are met.

Private health insurance

Each person usually needs a contract and a premium of their own.

When care is needed

Statutory health insurance

Billing often runs directly with the fund; co-payments at defined points.

Private health insurance

An invoice is often received, submitted, with the deductible and any unreimbursed share carried separately.

Over the years

Statutory health insurance

The contribution moves with income and system rules; switching has conditions.

Private health insurance

The premium can change; ageing reserves are not a promise to stand still. A return to statutory cover is often narrow.

CapitalPath does not treat statutory cover as a shortfall or private cover as a step up. Orientation starts with work, family and what can be carried for decades.

Access

Who can generally consider private health insurance.

Not everyone in Germany can leave statutory health insurance or start outside it. Access depends on the kind of work, sometimes on a statutory threshold, and on the insurer accepting the risk.

CapitalPath does not print a euro figure for the compulsory-insurance threshold. What matters is the applicable threshold, not a figure remembered from an older year.

Employees

A switch comes into view only above the compulsory-insurance threshold when other conditions are met. Below that, statutory insurance remains the frame.

Self-employed people

Many can consider private cover even below an income threshold. The premium must still be bearable in a thin year.

Studies and defined groups

Tariffs for students and narrowly described groups — not a general “cheap private plan for young people”.

Health at application

The insurer underwrites the risk; loading, exclusion or decline are possible. This page does not ask for diagnoses.

Life stages

The same insurance meets different lives.

Choose the closest stage. The note frames the question; it does not choose a tariff.

Life stage

Choose a stage. Orientation, not a suitability test.

Compulsory-insurance threshold, family, invoices and a premium that can be carried for decades. An employer subsidy shares the premium; it does not make the system a fit.

Missing statutory sick pay is often the quieter point. Treatment and income protection remain two contracts.

A student tariff as a bridge when conditions are met — not “the cheap private plan”. After graduation: ageing reserves, switching and health checks matter.

Each child in private cover is usually a premium of its own. Family planning and a wish for family insurance often point to statutory cover — fit, not a shortcoming.

The premium continues when earned income stops. Ageing reserves and relief agreements are mechanisms, not a promise of an unchanged premium.

Orientation

Fields and comparison points before a tariff name.

Six fields — more useful than stars in a table.

  1. 01

    Outpatient

Path to a doctor, medicines, remedies, aids, vision, prevention — the path to a specialist can change the rate.

  2. 02

    Hospital

Choice of clinic, treating physician, room; a single room and chief physician are often separate layers.

  3. 03

    Dental

Treatment and prophylaxis differ from tooth replacement; scales limit the early years.

  4. 04

    Deductible

Amount, areas, exceptions — a lower premium is the price of that gap.

  5. 05

    Premium over time

Ageing reserves, relief in later life, family, a return to statutory cover, a move.

  6. 06

    Treatment abroad

Europe and the rest of the world are separate; ordinary residence and duration are the limits.

Only now: an insurer

DKV BestMed is an architecture, not a verdict.

CapitalPath stays orientation. DKV appears here because April and May 2025 materials describe these tariffs — not a ranking and not a suggestion to apply.

BestMed Eco · BME

Structured basic private cover with a GP pathway. Percentages often depend on following that path.

BestMed Comfort · BMK

Free choice of outpatient physician, double room and private physician. Optional layers for dental materials, single room and Best Care.

BestMed Uni · BMUNI

For students and a group described in the portfolio — a bridge, not “cheap private cover for everyone”.

PremiumMed (April 2025 portfolio: PMN and PMA) is another level. No detailed PremiumMed brochure is part of this page — so no PremiumMed benefit figures are published here.

DKV · BestMed Eco · BME

Eco follows a GP pathway.

Figures from the English BestMed Eco brochure, April 2025, and where needed from the April 2025 portfolio. Conditions stay inside the sentence.

Outpatient

100% for the GP as the tariff defines that role, including ophthalmologist, paediatrician and gynaecologist. A specialist after a first visit to the GP: 100%. Straight to a specialist: 75%, and 100% if the GP later confirms the need for further specialist treatment. Medicines and dressings: 100%. Vision aids up to €200 within 24 months.

Practitioner, alternative care, aids

Non-medical practitioner: 75% up to €500 per coverage year under the GebüH. Alternative medicine on the Hufeland list: 75% up to €500, or 60% up to €500 without the GP pathway. Listed aids: 100%. Further aids: 100% through the aid service, otherwise 75%, following the brochure overview and the April 2025 portfolio.

Psychotherapy

After prior approval by DKV: 100% for the first 30 sessions, 80% from the 31st, 70% from the 61st. Without the GP pathway: 75%, 60% and 50% on the same tiers. The rate table does not cap the number of sessions.

Dental

Dental treatment, including targeted check-ups and professional cleaning: 100%. Tooth replacement and inlays: 70%. Implants: 70% for at most four per jaw, counting implants already present. Orthodontics: 90% if treatment starts before the 19th birthday. Materials and laboratory within the price and services schedule.

Dental scale

For tooth replacement including inlays, implants and orthodontics: up to €1,000 in the first coverage year, €2,000 in the first two, €3,000 in the first three, then at most €3,000 per coverage year from the fourth year. The overview and the portfolio do not describe the accident limit in the same way. This page therefore publishes no single accident cap.

Hospital

100% of general hospital services, treatment by the hospital physician and not the chief physician, a shared room, free choice of hospital including private clinics. A compensatory daily hospital allowance is not included in BME. From the 92nd day of an inpatient stay, a premium exemption can apply under conditions.

Deductible

BME1: €650, or €300 for children and adolescents. BME2: €1,200, or €600 for children and adolescents. The brochure describes it as applying to outpatient medical treatment, with a yearly right to switch. Flat-rate payments are described separately.

Fees and prevention

Physician and dentist services within the maximum rates of the fee schedules. The portfolio names the factors 3.5 / 2.5 / 1.3. Prevention under statutory programmes: 100% with the GP pathway, otherwise 75%, without travel vaccinations. Neither is outside the deductible.

DKV · BestMed Comfort · BMK

Comfort describes free choice and a different hospital layer.

Figures from the English BestMed Comfort brochure, April 2025. Where a highlight and the overview in that same brochure disagree, the sentence says so.

Outpatient

100% for outpatient treatment and examination with free choice of physician. Medicines and dressings: 100%. Vision aids up to €500 within 24 months. Remedies: 100% up to the maximum amounts in the tariff list.

Practitioner, alternative care, aids

Non-medical practitioner: 80% up to €1,000 per coverage year under the GebüH, including prescribed items. Alternative medicine on the Hufeland list: 80% up to €1,000, performed by a physician. Listed aids: 100%. Further aids: 100% through the aid service, otherwise 80%.

Psychotherapy

After prior approval by DKV: 100% for the first 30 sessions, 80% from the 31st to the 60th, 70% from the 61st. The rate table does not cap the number of sessions. The Comfort overview does not add a GP pathway to this tier.

Dental

Dental treatment, targeted check-ups and professional cleaning: 100%. Tooth replacement and inlays: 75%, or 85% with a dentist cooperating with DKV. Implants: 75% for up to six per jaw, counting those already present, also 85% with a cooperating dentist. Orthodontics: 100% if treatment starts before the 19th birthday. Materials and laboratory under the price and services schedule.

Dental scale

For tooth replacement, crowns, inlays and implants: up to €1,000 in year one, €2,000 across the first two years, €3,000 across the first three, €4,000 across the first four, €5,000 across the first five. From the sixth coverage year the overview says the scale no longer applies, and the same for accidents. Orthodontics is described separately: 100% if treatment starts before the 19th birthday. This page does not claim that the scale also covers orthodontics. The BMK scale is not used to calculate benefits under the BMKD upgrade.

Hospital

Free choice of hospital, including private clinics. 100% of general hospital services, 100% for a private physician including the chief physician, 100% for a double room. A single room at 60% if the double room is a general hospital service. Compensatory daily hospital allowance, as the portfolio states it: €20 if the double room is waived, €60 if the private physician is waived. The brochure summarises this as up to €80 a day if private-physician care and/or the double room are waived. A premium exemption from the 92nd day, under conditions.

Deductible

It applies across benefit areas, with a yearly right to switch. BMK0: €0. BMK1: €400, or €200 for children and adolescents. BMK2: €960, or €400. BMK3: €2,000, or €920. Not counted, among other items: prevention, vaccinations, the childbirth flat rate, the outpatient-surgery flat rate, dental prophylaxis including professional cleaning, and the compensatory daily hospital allowance.

Fees and prevention

BMK may reimburse above the maximum rates of the fee schedules when the tariff conditions are met. The overview says “under certain conditions”, the highlights mention verified complications, and the portfolio says medically justified and reasonable. The conditions decide which test applies. Prevention beyond statutory programmes, according to the list, at 100%, and outside the deductible. Travel vaccinations are described in BMK.

DKV · BestMed Uni · BMUNI

Uni is a bridge for a described group.

Not “private cover, only cheaper”. The benefit structure is in the May 2025 brochure. The group of people, and the calculation without ageing reserves, are in the April 2025 portfolio.

Whom the portfolio describes

Pupils from the age of 15. Students enrolled at state or state-recognised universities, and comparable universities in the EU or EEA, with ordinary residence in Germany when the contract starts, who have not yet reached the maximum entry age. Spouses and children who are dependants and have no income of their own. The portfolio does not state that maximum entry age. CapitalPath does not add a number.

After studies

The brochure describes Uni as a gateway to BestMed Eco, BestMed Comfort and PremiumMed after studies, without a renewed health check and without a waiting period, under the tariff conditions. The portfolio adds: eligibility for the new tariffs, those tariffs open to new business, the change requested within two months, and the new cover follows on directly. After a temporary period of unemployment the tariff can continue, also if requested within two months. That is a path with conditions, not an automatic stay on the student tariff.

How it is calculated

The portfolio describes BMUNI on a life-insurance basis, without ageing reserves. That is the plain reason to take the bridge seriously. It is not a hidden discount.

Outpatient care and psychotherapy

The same GP logic as Eco: 100% with the GP, 100% with a specialist after that first contact, 75% when going straight to a specialist, and 100% if the GP then confirms further treatment. Vision aids up to €200 in 24 months. Further aids: 100% through the aid service, otherwise 75%, following the brochure overview and the April 2025 portfolio. Psychotherapy after prior approval: 100% / 80% / 70%, or 75% / 60% / 50% without the GP pathway.

Dental and hospital

Dental treatment and prophylaxis, including professional cleaning: 100%. Tooth replacement and inlays: 70%. Implants: 70% for at most four per jaw. Orthodontics: 90% before the 19th birthday. The scale matches Eco: €1,000 / €2,000 / €3,000 in the early years, then at most €3,000 a year. Hospital: general services, the hospital physician and not the chief physician, a shared room, free choice of clinic. No compensatory daily allowance inside BMUNI itself.

Deductible

€300 per calendar year for outpatient services. The childbirth flat rate and the outpatient-surgery flat rate are not counted. The portfolio also says the deductible is not applied to childbirth.

A simplified overview. The insurance conditions, tariff terms and underwriting decide the actual cover. Empty or conflicting points are left unresolved on purpose.

Optional layers

BMKD, BMZ1 and Best Care are not inside the base tariff.

Only what the 2025 brochure or portfolio actually says.

BMKD · dental upgrade on Comfort

Supplements BMK 0–3 up to customary prices in Germany for the shortfall from the price and services schedule, at the same percentages as BMK. The BMK dental scale does not apply to BMKD. The portfolio describes BMKD with no deductible. BMKD belongs to the Comfort range, not to Eco or Uni.

BMZ1 · single room

An upgrade layer. With BMK, the portfolio describes BMZ1 as 100% of the extra cost between a single and a double room, and 100% for a single room if the double room is a general hospital service. Waiving the single room: €20 a day. With Eco and Uni, a single room is only together with the double-room and private-physician layer. The Uni brochure says that explicitly for the single-room module and for Best Care.

BMZ2 · double room and private physician on Eco or Uni

For BME and BMUNI, the layer that adds a private physician and a double room. A single room (BMZ1) requires that layer. Comfort already includes a double room and a private physician in BMK.

KBCK · KombiMed Best Care

An optional layer, not part of BME, BMK or BMUNI. For defined serious illnesses DKV arranges an appointment with a cooperating expert within five working days after the documents are in: admission, or a pre-inpatient consultation. The treatment costs themselves are not covered by KBCK. The rate requires inpatient cover with a double room and a private physician. Cover applies within Germany. A general waiting period of three months, which does not apply to accidents.

Premium, deductible, refund

Three amounts. Only one is entered locally.

A lower premium, a higher deductible and a premium refund are not the same money. The board and an independent trustee set the form and the amount each year. A year without outpatient or dental claims, with exceptions for prevention, is a condition in the brochures. The 2025 documents do not describe the link to company performance in the same way. This page adopts neither wording. The euro amounts below are historical examples of the payout in 2025 for financial year 2024, not a promise for a later year.

Payout in 2025 for financial year 2024

Historical examples, not a forecast. The amounts can change. They are not a promise for a coming year, and they are not a quoted premium.

  • BMK0 to BMK3

    €2,100 / €1,800 / €1,300 / €900. The brochure’s figures for children and adolescents, after the slash: €1,050 / €900 / €650 / €450.

  • BME1 and BME2

€1,200 and €900. For children and adolescents, €400 and €300.

  • BMUNI

€660 for adults, €330 for children and young people.

Treatment abroad

Europe and the rest of the world are different sentences.

Always with the tariff. Ordinary residence in Germany is the condition the brochures attach to the periods they describe. Moving country is not the same as a trip.

  1. Inside Europe

BME, BMK and BMUNI describe cover for a temporary stay as unlimited in time if Germany is the customary place of residence. For Eco and Uni the brochure speaks of benefits equal to customary costs of treatment in Germany. For Comfort the highlights speak of customary costs in that country. Those are not the same sentence.

  2. Outside Europe · BME

BestMed Eco (BME): one month, plus up to two further months if a return cannot be made for health reasons. Benefits up to customary costs in Germany. Germany remains the customary place of residence. The April 2025 brochure and portfolio say the same thing here.

  3. Outside Europe · BMK

BestMed Comfort (BMK): the April 2025 portfolio and the highlights in the April 2025 brochure describe six months, plus up to two further months if a return is medically impossible. Benefits up to double customary costs in Germany. Germany remains the customary place of residence.

  4. Outside Europe · BMUNI

For BestMed Uni, the May 2025 brochure and the April 2025 portfolio do not state the same duration or the same cost limit outside Europe. Until that is settled, this page publishes no figure. Cover still depends on the tariff, and on Germany remaining the customary place of residence.

  5. Evacuation

The brochures describe a medically necessary evacuation arranged by DKV, up to 100% of the extra costs, otherwise less. That is a condition of how it is arranged, not an open ticket for every journey.

Where two 2025 passages do not say the same thing, the conflict stays visible. It is not averaged into a smooth number.

Further topics

Digital pathsApps and phone lines depend on the tariff and on the conditions.

My DKV, TeleClinic, the DKV Health Line and case management appear in the 2025 brochures. They are not automatic in every tariff, and not the same entitlement for every question.

My DKV

The Comfort brochure describes the app for organising the contract. That is a service channel, not a benefit promise.

TeleClinic

Described as a partner’s digital consultation, for DKV policyholders. Whether and how it applies in a given tariff is not a blanket yes on this page.

Health Line and the travel line

Advice, and a 24-hour emergency line when travelling, are named as services. A phone number does not replace the tariff’s scope abroad.

Case management and the aid service

The brochures mention support after certain events, and a path for health aids. Some reimbursement rates apply only if that path is used.

Digital health applications

DiGA on the official list: under BMK, 100% up to the statutory reimbursement, and further applications that meet the rate, up to €500 per calendar year. The Eco and Uni highlights name the listed applications without that same extra amount. Not every tariff carries the same figure.

The long premiumA premium that fits today is not yet a retirement calculation.

CapitalPath does not say that premiums stay stable. The brochures use language about later life. This page does not repeat it as a promise.

Ageing reserves

Many full-cover tariffs — in the portfolio, BME and BMK on a life-insurance basis — set part of the premium aside. That can soften the rise in later life. It does not prevent adjustments. BMUNI is described in the portfolio without ageing reserves. That is why the path after studies is its own subject, not a footnote.

Premium relief, VV65

The portfolio describes a voluntary agreement for people insured under BME, BMK, PMN or PMA. Entry from age 20 to 59, relief from the month after the 65th birthday, in steps of €50, at most the premium then in force. The duty to pay can continue past 65. Adjustments are possible with the trustee’s consent. A mechanism, not a fixed premium in later life.

What still changes

Medical costs, the tariff, and life circumstances. Children, part-time work, self-employment, a move, or a wish to return to statutory insurance weigh more than a rate that looks favourable today. An employer subsidy ends when the job ends.

Income during illnessDaily sickness allowance is not inside the medical tariff.

The physician’s invoice and a missing salary are two different gaps. The 2025 brochures treat daily sickness allowance as its own cover, beside medical-cost insurance.

Treatment costs

Missing income

A separate policy

The portfolio lists KombiMed daily sickness allowance as its own type, including for employees, self-employed people and freelancers. This page states no waiting period, no amount and no tax effect. There is no single figure here presented as generally valid.

What the brochures do make clear

The allowance requires a complete inability to work because of illness or accident, continues on Sundays and holidays, and is described as unlimited in duration. It ends with retirement. Whether it fits depends on income, the waiting period, and what statutory or contractual cover already exists.

Long-term care, briefly

Care is another layer. The portfolio names a compulsory long-term care tariff beside BestMed and PremiumMed. The benefits of those care tariffs are not set out here. Private care supplements are again different from treatment costs.

Questions

Questions people often ask.

Is private health insurance better than statutory cover?

No, not automatically. They are different systems. Statutory insurance often carries a family through family insurance, and it ties the contribution to income. Private insurance is a contract, with its own health questions, its own invoices, and a premium that does not fall when salary falls. Fit beats the word “better”.

Is private health insurance open to everyone?

No. Employees generally need income above the applicable compulsory-insurance threshold. Self-employed people have a different route in. Students only within the limits a tariff describes. The insurer has to accept the risk. This page deliberately prints no euro threshold, because the figure changes.

What is a deductible?

The amount carried in a year before, or while, the tariff pays. Under BestMed Comfort it applies across benefit areas. Under BestMed Eco and Uni the brochures describe it mainly for outpatient care. Prevention can sit outside it. It is the lever on the premium, not a discount without a consequence.

Is the premium refund guaranteed?

No. The board sets the form and the amount each year with an independent trustee’s consent. A year without outpatient and dental claims, with exceptions for prevention, is a condition in the brochures, not a promise. Euro amounts on this page are the payout in 2025 for financial year 2024, labelled as a historical example.

How do BME, BMK and BMUNI differ?

Eco (BME) and Uni (BMUNI) use a GP pathway, a shared room and the hospital physician. Comfort (BMK) describes free choice of physician, a double room and a private physician. Uni is calculated for a described group and, in the portfolio, without ageing reserves. The table on this page stays with the cells that have a source.

What does the GP model mean?

Under BME and BMUNI the full rate for a specialist applies if the GP, as the tariff defines that role, treated first or confirms further treatment. Otherwise the rate is lower: 75% for the physician visit in the overview, and different again for psychotherapy and some alternative methods. Ophthalmologists, paediatricians and gynaecologists count as GPs in these tariffs.

Are children covered automatically?

Not in private health insurance. Each child usually needs a contract. Statutory family insurance is a different mechanism. If several children, or a partner without income, are part of the picture, the private system can still be the wrong system even when a single benefit looks strong.

Does the premium stay the same in later life?

This page does not say that. Ageing reserves and an optional relief agreement are mechanisms. Adjustments remain possible. BestMed Uni is described in the portfolio without ageing reserves. A premium chosen today is not a promise for retirement.

Does cover abroad apply automatically?

Only within the tariff, and usually while Germany remains the ordinary place of residence. Europe and the rest of the world are separate. For BestMed Comfort, the portfolio and the highlights describe six months outside Europe, up to double customary costs in Germany. For BestMed Uni, the 2025 documents do not state the same duration or cost limit. That is why no Uni figure is published here.

Is daily sickness allowance included in BestMed?

No. It is a separate insurance beside treatment costs. BME, BMK and BMUNI do not replace a salary.

The other answer

When private health insurance may not fit.

A single benefit can look attractive. The system can still be the wrong frame for the situation. Saying so is part of orientation.

A benefit can be attractive — the overall system can still be the wrong frame.

Family planning

Several children, a child on the way, a partner without their own income. Separate premiums add up. Statutory family insurance is often the closer frame.

Decades

The premium must remain bearable when income falls — not only in the year of signing.

Health and underwriting

What already exists can make the contract more expensive, exclude something, or stop it. A conversation can look at that; a form on this page should not.

Wanting a way back

The route into statutory insurance is often narrow later. Leaving that door open belongs in the decision before switching.

Income in retirement

The premium does not end when salary does. The relevant figure is expected retirement income, not an employer subsidy today.

Invoices and the deductible

Without willingness to pay upfront, keep receipts, or carry a yearly gap, a private contract adds administrative work.

Benefit figures: DKV BestMed Eco BME brochure, April 2025; BestMed Comfort BMK brochure, April 2025; BestMed Uni BMUNI brochure, May 2025; DKV product portfolio, April 2025. Older conditions are not used to override a later figure. Not an offer and not personal advice.

Contact

Place the situation — appointment, call or message

Bring questions, not diagnoses. Which system fits shows up in a conversation and in the conditions — no application starts on this page.
